Dr. Shivani Sachdev Gour on NDTV News to Discuss How Child Marriage Impacts Girls' Lives

Asmita Dubey (Anchor) NDTV News: "Hello and a warm welcome to all of you. I am Asmita Dubey. As you can see behind me, what have we written? We have with us Dr. Shivani Sachdev Gour. Dr. Shivani, child marriages often lead to not only physical problems but also mental distress. Marrying at such a young age not only affects the current generation but also poses challenges for future generations, doesn’t it?"

Dr. Shivani Sachdev Gour: "Absolutely correct, Jyoti. Physically and mentally, a girl under the age of eighteen is never ready for this responsibility because her development is not complete at this stage. The reason it is so dangerous is that studies, especially those on maternal mortality (the death of a mother during pregnancy), have shown that the highest death rate is found in girls who get pregnant under the age of eighteen. This is a United Nations developmental goal, and it reflects on the development index of an entire country, which includes the maternal mortality rate.

Pregnancy at this age is the most dangerous time for a girl, and it is crucial to emphasize and understand this. For instance, anemia is very common among these girls, with over fifty percent being anemic. Pregnancy in such cases increases the risk of miscarriage, abortion, and infections like sepsis, which can spread beyond the uterus and become life-threatening. Another concern is bleeding, which we call antepartum hemorrhage, and conditions like eclampsia or pre-eclampsia where blood pressure rises, leading to severe complications.

Dr. Shivani Sachdev Gour Discusses the Disappearance of the Y-Chromosome

About two years ago, a study was published revealing that in two rodent species — the Eastern European wolf and the spiny rats of Japan — the Y chromosome has disappeared. The research also indicated that these two rodent species have developed a new sex-determining mechanism in another chromosome, where the SRY gene, which typically determines male sex on the Y chromosome, has been relocated. Despite the disappearance of the Y chromosome in these species, males are still being produced because the sex-determining function has been transferred to another chromosome.

In humans, the Y-chromosome is about one-third the size of the X chromosome and contains around 100 genes. In comparison, the X chromosome, which is present in females, has about 900 genes. Over the years, approximately 90% of the genes on the Y chromosome have been lost, and now only about 55 genes remain. It is estimated that it will take another 10-11 million years for the Y chromosome to potentially lose all of its genes.

Question: What complications can arise if girls do not use sanitary pads, and how significant is the impact of this decision by the Madhya Pradesh government?

Dr Shivani Sachdev Gour: अगर हम सुरक्षित स्वच्छता प्रथाओं का पालन नहीं करते हैं, तो कई प्रकार की जटिलताएँ हो सकती हैं, Like Infection, vaginal infection, urinary infection, irritation etc.

Why Age Matters in Egg Freezing!
Age has an important impact and between mid 40s to late 40s the fertility is almost non-equal. Some patients before 30 years For example, if they have been diagnosed with cancer or endometriosis, they come in between 25 to 30 years to get their egg freezing done.

Understanding Hormonal Injections in Egg Freezing
The injections of hormones take every hours because its effect is very temporary, there is no long lasting side effect. So, this medical point of view is the biggest concern. The second concern is that will the freezing cause damage to the egg? Will there be any genetic problem in the egg freezing? So, we want to clarify the myth of this that if you take out the egg after freezing, it literally looks like it has been bathed.

How Does Age Affect Egg Quality?
With age, the quality of the eggs changes a lot. This is called a biological clock of a woman. from the age of 30 to 35, the best age is for you to do egg freezing. After 35 years, the number and quantity of eggs decrease.

Debunking Myths About Egg Freezing
I want to clarify all these myths that this is not at all true. If you freeze 10 or 12 eggs, then you will not be able to become a mother in the future. you will not be able to become a mother naturally and your eggs will be finished.

How Many Eggs Do You Need for Pregnancy?
10 to 15 is considered an ideal number that you have an expectation of success in that. If you get it done at the age of 30, then once you get it done, you will get 10 to 12 to 14 eggs. But if you do it after 40 or after that, then you may have to repeat the whole process 2 to 3 times and whether you get 15 eggs or not is doubtful.

The Biological Clock and Egg Freezing
As time goes by, the biological clock, according to that, they may have problems in getting pregnant later. And you know, our inner desire is, the desire of every cell of our body is that we want our own egg, we want our own child.

Family Support in Egg Freezing Decisions
Sometimes you get support from the family. Sometimes you don't get support. You have to bear the financial patient yourself. But it is not guaranteed that you will get pregnant from those eggs. And if you get pregnant at an older age, diabetes, high blood pressure, abruption, bleeding in pregnancy, you may have more complications.

Understanding the Costs of Egg Freezing
So, if we give them the full amount, then in one cycle, it can be somewhere between 1.5 to 1.7 lakhs. And in this, freezing will initially be included, but when the eggs are frozen for a longer duration, then the freezing charges will depend on how many eggs we have, how many leaves we have So that can vary from center to center between 30,000 to 50,000 and annual charge that is to be incurred.

How Innovations and Regulations Impact the Future of IVF Treatment

Dr. Shivani Sachdev Gour, Director of SCI Healthcare, discusses IVF. India's young population has driven up demand, with over 1 in 10 couples facing conception issues. “While surrogacy for international patients is banned, many still come from SAARC and African countries for IVF. The industry has become more organized, with new centers and corporate IVF chains,” says Dr Shivani Sachdev Gour.

“India currently conducts an average of 200,000 to 250,000 IVF cycles per year, with projections indicating this could rise to 500,000 to 600,000 cycles annually."

She also predicts that the market will likely see the entry of new players as well as numerous mergers and acquisitions. Smaller local players may be acquired by larger companies, a trend similar to that seen in the hospital sector, which will lead to greater transparency and higher quality treatments for patients. She notes that major brands are aiming to expand their reach to tier 2 and tier 3 cities.
