WHO 2010 guidelines for semen evaluation suggest a TMSC of 9 million atleast for a natural pregnancy. A large meta analysis of over 1000 patients evaluating impact of varicocele repair on ART outcome in OLigospermia and azoospermia has shown men who have surgery are 1.7 times more likely to have a pregnancy as compared to men who did not have surgery. Varicocele surgery can have a significant impact on fertility outcome and can upgrade couples from iui or ivf to more natural pregnancy options.
